If this is difficult, please fill it out on the day of your visit. Please bring your health insurance card, your miner's card, and any othernecessary documents such as workers' compensation. If you are referred from another hospital, please bring your medical information form, imaging data, and other test results. ■New patient appointments cannot be made for third-party injuries suchas traffic trauma.
