| First consultation |
| Our clinic mainly treat musculoskeletal injuries and diseases. You can make an appointment on the web for your first visit. You can see a doctor without an appointment. It will go more smoothly if you fill out the medical application form（PDF file） in advance. Please bring your health insurance card or my number card. Please contact us in advance for traffic accidents caused by a third party. Health insurance cannot be used in the case of work-related accidents. Please fill out the prescribed form and bring it with you. Payment can be made in cash or by credit card. If you are contacted by a hotel or other accommodation staff, please provide your name, age, gender, and a brief description of the injury. |

| No medical insurance of Japan, 30 yen per point. *This point is defined as medical insurance by the Ministry of Health, Labor and Welfare of Japan. *Medical Certification in English, 12,000 yen per one. |
