Who Can Book

  • First-time visitors to our clinic
  • Patients whose previous treatment has been completed and some time has passed

* Please download the patient registration form (PDF), fill it out, and bring it with you. You may also fill it out at the clinic on the day of your visit.

* Please arrive at least 5 minutes before your appointment time.

* If you are being referred from another clinic, please bring your referral letter and any test results (e.g. imaging data).

* Our clinic operates with two physicians, ensuring quality care and shorter wait times.
